Application Forms
If you live in Ireland application forms are available from most Post Offices, all Garda Stations, many Citizens Advice Centres and Libraries and also from the Passport Offices in Dublin and Cork. Forms are also available from Irish Diplomatic and Consular Missions overseas.
Copies of these application forms are shown for viewing purposes only.
Applicants in Ireland who are unable to call in person to or who require large supplies of forms can also obtain the relevant application by contacting the Passport Offices here.
New format application forms have been introduced from 1 October 2006. APS 1 form is for use within the State. APS 2 form is for use by persons resident outside the State. Applicants are reminded that the APS1 and APS2 forms are shown here for illustration purposes only. Please obtain application forms from one of the outlets mentioned above.
The current form is a simplified one which incorporates the application section, parental consent [for persons under 18 years of age] and statement of loss [where a passport is lost or stolen] into a single form.
There is an additional SOLE GUARDIAN AFFIDAVIT (Word 30kb) form which must be submitted where a parent claims to be the sole guardian of a child seeking a passport. An Irish version of the Sole Guardian form may be downloaded here (PDF 412kb).
PASSPORTS FOR CHILDREN BORN ON OR AFTER 1 JANUARY 2005
The passport entitlement of any child born on the island of Ireland on or after 1 January 2005 is governed by the citizenship of a parent or the residency history of one of the parents at the time of the person's birth. Each passport application for children born in Ireland on or after 1 January 2005 must have additional evidence of citizenship or residence from a parent. In summary:
- for a child with an Irish parent the Passport Office requires a birth cert or passport of that parent (birth cert is preferable)
- for a child with a British parent - their passport
- for a child with an EU parent - their passport and also Declaration Form A, downloadable from the website of the Department of Justice Equality and Law Reform (PDF 283kb) for a child with parents of another nationality - the parent's passport with immigration stamps showing full lawful residence for 3 of last 4 years
- Children born in Northern Ireland whose parents are non Irish/British/EU nationals are required to obtain a Certificate of Citizenship from the Department of Justice, Equality and Law Reform before making a passport application.
